Responsibilities and Duties Responsible for coming in time & maintaining the roaster by mutual adjustment. In case of any special offs / adjustments, a pre-information to Administrator should be given. Responsible for the patient’s treatment & well-being from admission till discharge, i.e. they should try to provide effective & prompt treatment so that the patient should leave the premises satisfactorily. Proper Documentation i.e. Daily Notes on Progress Reports. The doctors mention all the procedures done on the case sheet. In case any patient gets shifted from ICU to ward, a ‘Detailed Transfer Summary’ mentioning Time & Date of transfer should be written by the ICU Resident. Proper consent should be taken & checked (if taken by staff). In case of any serious patient / poisoning etc, detailed Risk Consents in Hindi for M.L.C & Treatment & Risks involvement should be taken by the Doctor. The Doctors are responsible for rounds with consultants, maintaining the Round Register & note the details of the consultant’s or visiting consultant’s orders. They should also check that the orders should be carried out by the staff in the prescribed manner. Will listen to patient’s complaints and see that they should be solved as soon as possible. At every complaint, the resident doctor is supposed to go & check the patient. He should try to solve the patient’s problem by giving medicines or by counseling, if possible. If on examination it is found that the patient is not comfortable or in a bad state, he should inform the respective consultant immediately. If there is no serious problem, but the patient wants to talk to the consultant, the Resident Doctor should introduce himself to the patient and try to solve the problem and should inform the consultant about that. In case of any patient with chest pain, ECG should be taken immediately & the patient should be kept in observation for some time. ECG should be shown to ICU Resident / Junior Consultant Only after consultation with the Senior Doctors, the patient should be sent home / referred to the other hospital. In case of Routine Admissions, the patient should be given an estimate about the disease, line of treatment, hospitalization period. If the patient is convinced, only then the patient should be admitted. Ideally (except in special circumstances), advance amount should be deposited at the Reception Counter. Duty Doctor in Emergency / ICU are responsible for checking that Resuscitation set, Emergency Tray, ENT sets, LP sets, Dressing sets, POPs, Splints, Stitching sets, Lavage sets, Catheter sets & Gloves etc are proper and available in time. In case the persons responsible for these things are not doing the things properly, they should be instructed for the same and in case of repetition of mistakes, a signal should be raised & information should be sent to the Administrator. In case of any System Break Down / Patient’s Complaint, the Administrator should be informed immediately. They are responsible for rounds, preparation of discharges, producing Medical Examination Certificates & looking after the grievances of the patients of the hospital in total irrespective of the place of deputation i.e. they should cooperate with each other. In case of need of emergency help, it should be provided by everyone. The residents are responsible to discuss about charges with the respective consultant (in case of unusual cases) and give the estimate to the patients before the procedure. A special hand-over take over register should be maintained at each place and if possible, bed to bed over should be given in detail. In case of Brought Dead Patients: - For stitching, the patient should be shifted to Emergency Minor O.T & the attendants should sit outside the emergency. Every minor / major stitching should be done under strict asepsis & cut sheet should be used after washing the wound, then the stitching should be started. Every stitching on face will be done by 3-0 or 4-0 silk. If the patient wants to get it done by plastic / cosmetics Surgeon or General Surgeon, they should be called with prior information charges to the patient. Shiftings - The Doctor on Duty should see that the patients should be shifted properly according to the protocol in the respective accommodation / ambulance. Doctors should try that the patient should be immediately shifted from emergency as soon as he get settled. Every patient arising in emergency hours should be paid proper attention and every possible measure should be adopted to make the patient comfortable and happy.
