War Memorial Hospital (WMH) is a 3rd Schedule Aged Rehabilitation hospital, situated in Waverley, owned and operated by Uniting and part of the South Eastern Sydney Local Health District (SESLHD). Services are comprised of 24 hour inpatient care, Monday to Friday outpatient departments, several specialist community teams inclusive of Transitional Aged Care, Aged Care Assessment Team, Geriatric Flying Squad, Young Onset Dementia, 7 day a week day centre, 24 hour accommodation service for regional guests and the full range of hospital support services. It is set within beautiful tranquil grounds and is a well-supported team environment. This Social Worker Level 3 position sits within a multidisciplinary team offering coordinated rehabilitation services and discharge planning for older people. Objectives of the role are to: Provide in-patients and their families and carers with high quality, person-centred support during their stay; Plan and coordinate appropriate services for discharge in conjunction with multidisciplinary team; Work towards quality improvements in social work and multidisciplinary services in line with War Memorial Hospital’s overall objectives. This Social Work Level 3 position sits within the WMH Social Work Department and works predominantly as part of the inpatient team. This role may also be required to work in the outpatient and/or community setting as requested and directed by Social Work Manager. Demonstrated ability and commitment to initiate and lead quality improvement initiatives with other staff and a commitment to ongoing clinical and professional learning. Qualifications: Qualifications in accordance with the WMH Health Professionals Agreement, and eligibility for registration with the Australian Association of Social Workers Current NSW Drivers’ licence Class C
