St Luke’s Hospice is the major provider of specialist palliative care to the people of Plymouth, West Devon and East Cornwall. St Luke’s Hospice has a community team, an inpatient unit, a hospital team, an End of Life Care Service and a clinical education department. The Social Care team work collaboratively with all teams and our external partners to provide a holistic service to patients and their families. We are looking for an experienced social worker to provide emotional and practical psychosocial support and advice to palliative patients, their carers and families. The Social Care team aims to provide person-centered support that builds on individual’s strengths, enables informed choices and promotes health, wellbeing and independence for as long as possible. The team also support patients’ family members both pre and post bereavement. The primary focus of the role is to provide direct social work and psychosocial intervention in relation to complex issues such as concerns for children and young people, issues relating to risk, vulnerability, mental capacity, best interest care planning and safeguarding. The social worker will hold a caseload of clients with a range of needs which may include mental health issues, learning disabilities, dementia, autism, carer issues and family conflict. The post holder will also support the development of in-house social care services and facilitate signposting and referral to and collaboration with external agencies as appropriate. We have a well-established integrated service and require a dedicated and compassionate social worker with post-qualification experience and a commitment and passion for teaching and supporting others.
