Signposting patients - across health, social and community services Solving problems - finding solutions, liaising with other health care professionals Educating patients about the most appropriate services to access, screening and applications to support self-management Undertaking questioning to determine the patients needs within the scope of the role and signposting on to other services where required Booking appointments using EMIS and SystmOne Contacting patients with advice from GP's which is received on task Covering Reception when needed. Familiar and have an understanding of Medical Terminology Working as an integral part of an established team Having an understanding of the Data Protection Act/GDPR 2018 Able to cover surgery hours on a shift basis (discussed at interview) To work in accordance to written protocols The right candidate must have 6 months experience within health and social care Experience of a person-centred planning Computer literate Excellent communication skills, both verbal and written, able to supply reports as required This post demands someone with a mature outlook, able to be supportive while being clear on boundaries Job Types: Full-time, Permanent Experience: Administration: 2 years (Preferred)
