Temporary Full-time for approximately 6 months Responsibilities: To provide clinical services such as drug therapy review and monitoring, individual and group patient counselling, in-service education, drug information, and participation on multidisciplinary patient care teams To work with the support of a computerized unit dose drug distribution system as a member of the Pharmacy team Demonstrate QCH values: accountability, respect, innovation Qualifications: Graduate of a recognized Faculty of Pharmacy and hold a Bachelor's Degree of Science in Pharmacy Be licensed or eligible for licensure with the Ontario College of Pharmacists Completion of a hospital residency or possess hospital experience, together with a background in a clinically oriented practice Demonstrated effective communication, interpersonal, and organizational skills Commitment to continuing education related to position
