The primary role of the PCA on the medical ward – is to assist in the delivery of a high standard of person centred care, work under supervision of the registered nurse, assist with the care requirements and hygiene needs of patients across the medical ward. Provide diversional tactics to redirect and engage patients with cognitive behavioural challenges. 2 RESPONSIBILITIES AND MEASURES OF SUCCESS IN THE ROLE The following table breaks down the key performance areas of responsibility for the incumbent. Measurements for performance areas will be agreed to with the Reporting Manager PERFORMANCE AREA RESPONSIBILITY Core Role To provide timely delivery of personal care to patients occupying the Increased Observation Area within the Medical ward at Northeast Health Wangaratta. To provide diversional therapy support to patients within the Increased Observation Area as required and directed by the Registered Nurse. To provide assistance with hygiene and toileting of patients. To support and assist with mobilisation and pressure relief to patients. To observe and interact with patients who are requiring a higher level of observation. To assist in the delivery and feeding of meals, beverages and in-between snacks and supplements to patients within the ward area. - To assist with the collection of patient trays, crockery and utensils following meal and in- between meal services. To ensure a clean, safe and hygienic environment is maintained to the required level at all times. To work as part of a team, supporting others in the delivery of evidenced based patient care. Follow task details specific to the work area in which you are allocated. Maintain all relevant documentation as required and determined by the area of work. Implement and uphold falls prevention strategies. Contribute to delirium supporting strategies. WORKING RELATIONSHIPS INTERNAL Nursing Staff Medical staff Allied health staff Support staff EXTERNAL Patients and Visitors SPECIFIC SKILL REQUIREMENTS / QUALIFICATIONS / QUALITIES Essential Be available to work a seven day a week rotating shift roster which is inclusive of weekends, day, afternoon and nightshifts.. Adhere to NHW confidentiality policy. Ability to uphold the Standards of behaviour and live the values of the organization and translate this to the core components of the role. Cert III individual support (Ageing) or Cert III in health support assistance (Acute and Aged Care) Desirable Experience working within an acute hospital environment Experience working with patients with delirium and/or cognitive impairment. Experience with diversional therapy tactics. All staff must have and remain current for continued employment the following: A current National Police Check (renewed every 3 years) A current Working with Children Check (renewed every 5 years) Statutory Declaration for applicable workers who have lived overseas 3
