An exciting opportunity has arisen within the Wellbeing IAPT Service Mental Health Practitioner team for a dedicated and outcome focused Mental Health Practitioner to join our multi-disciplinary team. The Wellbeing Service is an integrated treatment and recovery service which offers a range of psychosocial interventions to Norfolk residents age 16 years and over. The role of the Mental Health Practitioner includes triaging clients coming into the Wellbeing service as well as attending interface meetings between IAPT and secondary Mental Health Services. You will also be required to undertake screening, assessment, supporting client transition from primary to secondary care services, assist in the provision of on-call duty services, and have the opportunity to work with clients offering brief interventions on our Resilience Care Pathway. The post holder will work with other agencies, NSFT secondary care Locality teams within the Norfolk area to support service users to be signposted to the most beneficial service for their needs.
