TEAM LEAD – CARE NAVIGATION Full Time Position 37.5 hours per week Client & Family-Centered Care Is At The Heart of Everything We Do At Lumacare! The ideal candidate ideal candidate will have a friendly personality and will enjoy having direct contact with all kinds of clients whether they are frail, vulnerable, or elderly, while assessing their needs and helping them to plan for their care. He or She will also have the experience and knowledge to provide support to the Care Navigation Team in collaboration with the Client Care Manager Your role would involve: · Managing the Home and Community Care (HCC), Health Partners Gateway (HPS) referrals, wait lists and discharges. · Coordinating the caseloads of the Care Navigator staff and monitoring for quality of service; this includes leading the admission and discharge process for the team, working closely with the referral source. · Supporting the effective operation of Care Navigation services by ensuring reviews of client care plans. · Providing leadership and support in managing escalated client concerns and client safety event reporting. · Support program managers with monitoring targets and statistical reports and ensures team awareness and problem solving strategies · Assisting with the development, implementation, monitoring and evaluation all care navigation services. · Assisting in the implementation processes to support best practices as well as the delivery of safe, effective, efficient and quality services. · Promoting Lumacare programs and services through outreach, education and team meetings at partner hospitals, referral sources, senior buildings and stakeholder networking opportunities. · Collaborating with partners, representing the agency at internal and external planning meetings · Identifying training needs; developing and coordinating training based on staff needs. · Addressing concerns and complaints appropriately with team/client collaboration approach · Monitoring of targets and statistical reports. This position is a Permanent Full Time position that may require on call duties. Requirements for the position include: · A Social Work degree in good standing with the College of Social Workers and Social Service Workers; cultural competency training is an asset. · Minimum of five to seven years of experience within the community support service sector or health care environment. · Minimum of three years of case management/coordination experience including conducting assessments and interventions, care planning and monitoring, understanding evaluative processes as well as possessing knowledge of fiscal monitoring/reporting. · Have experience working with the frail and elderly population and knowledge of community resources including various funding sources. · Understand in-home health care needs and has familiarity with the MOHLTC Policy for High Risk Seniors. · Strong critical thinking skills as well as strong verbal and written communication skills. · Excellent assessment and problem solving skills, as well as leadership and coaching/mentoring skills. · Strong organizational and time management skills. · Have excellent computer skills with proficiency in MS Word, Excel, Outlook and Power Point; knowledge of CIMS is an asset. · Familiarity with the InterRAI-CHA assessment tool is a requirement. · A Valid Ontario Driver’s License and access to a vehicle. · Valid Vulnerable Police Record Check completed within last 12 months. · Current First Aid and CPR training. · Fluency in other language(s) is an asset.
