DEPARTMENT/AREA OVERVIEW: The Care Home at Harbour Landing Village delivers comprehensive and customized person-centred care 24 hours per day to address our residents’ varying needs. Adults who require greater levels of support will feel comfortable and safe in our community. Our onsite care professionals include: Licenced Practical Nurse Care Aides Medication Aides Therapists Personal Support Workers JOB DESCRIPTION SUMMARY: The Continuing Care Assistant, under the supervision of the Director of Care at Harbour Landing Village, is responsible for providing homemaking, companionship, personal care and personal assistant services to HLV residents and their families within a multidisciplinary approach to resident care. The Continuing Care Assistant supports residents with activities of daily living within a multigenerational culture that promotes community inclusion and programming. Continuing Care Assistants work closely with the rest of the HLV Care Team to follow resident care plans, provide emotional and physical support for residents and treat village residents and team members with empathy, kindness, compassion and respect. RESPONSIBILITIES/DUTIES: Provide homemaking services including, but not limited to cleaning, cooking, laundry and general household management Provide companionship and/or accompany residents to activities and events within the Harbour Landing Village community. Provide personal care service according to directions within resident-centred care plans Ensures that the emotional, spiritual, physical comfort and safety of our residents are met to the greatest possible extent Provide assistance with resident specific activities of daily living (personal assistance tasks) Perform specialized, resident specific procedures for which recognized training and current competency can be demonstrated Observe and document client conditions according to Harbour Landing Village Care Team standards; report unusual events or changes in the client’s condition to the immediate Supervisor or to the Office Understands and implements infection prevention practices. Participate in ongoing internal and/or external continuing education activities Adhere to Harbour Landing Village Employee and Care Team Policies and Procedures. Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify Director of Care of any Health & Safety risks or concerns. Maintains confidentiality of client and Harbour Landing Village information and discusses same only with appropriate HLV personnel Complete other tasks as requested SKILLS & ABILITIES: The flexibility and willingness to learn. Organized with strong attention to detail. Strong communication skills, both written and verbal. Interpersonal abilities with a team-focused ethic. Physically able to lift 23 kg and able to move freely Can assist clients with standing, walking, sitting and rolling in bed QUALIFICATIONS: CCA Certification and/or equivalent education and experience TLR, PART, Medical Module, First Aid and CPR EXPECTATIONS: Maintain employee and family confidentiality.
