The Intensive Case Management role is designed to respond to the gaps of the current mental health service system. The role will provide C2C participants with additional or alternative supports to those provided by C2C peer workers. Position background Ballarat Community Health (BCH) is the lead agency and partners with Grampians Community Health (GCH) to deliver this program in Central Highlands and the Wimmera. This program offers that people are experts in their own lives and should have the opportunity to play an active role in decisions that shape their lives. We also believe that by involving people with lived experience in the identification of challenges and development of solutions dramatically increases the chances of solutions to make a difference. Connecting2community is a co-designed model of psychosocial support delivered by qualified peer workers. The worker is supported by and connected to an established multidisciplinary team of mental health staff members led by the Manager of Business Services. The clients of Connecting2community may include some forensic clients but most will be from the broader community. The Intensive Case Management role will assist people who do not fit the criteria for the Peer Support Worker role, and offer additional support to navigate the mental health service system. Position Description Connecting 2 Community – Intensive Case Management The Connecting2community model enables clients to identify goals and gather assistance from their worker to link them into relevant services and activities in the community. These goals may relate to: social skills and friendships, managing daily living, budgeting skills, finding or maintaining a home, volunteering, education, physical health and building confidence and resilience. The model provides a time-limited intervention and is underpinned by the promotion of recovery and client choice and control. Eligible clients are adults with mental illness who are not eligible for the NDIS or are waiting for an access decision and their NDIS plan to begin. Key responsibilities 1. Support complex clients to test eligibility to the NDIS and develop NDIS applications. 2. Provide short term care coordination services to complex C2C clients to enable clients to access the most appropriate care and treatment pathways. 3. Provide short term psychosocial support to clients presenting with indications of risk of relapse and/or functional decline. 4. Provide secondary support to C2C peer workers which includes identifying appropriate referrals pathways. 5. Engage clients and develop trusting and professional relationships that respect worker/client boundaries. 6. Complete wellness plans and outcome star with clients. 7. Increase identified community groups and service providers to provide a welcoming and respectful environment to clients wanting to connect. 8. Contribute to the development of a day programs in support with BCH and GCH clinical leads and group programs. 9. Cooperate closely with team members in order to ensure continuity of care for clients. 10. Contribute to provision of groups for the client group in the community. 11. Actively participate in reflective practice and skill development through team meetings, service planning sessions, supervision and professional development activities. 12. Contribute to the further development of best practice, client-centred care and co-design practice by informing evaluations and consultations with clients and carers. 13. Collect, collate and maintain data for performance reporting requirements. 14. Follow leadership and management direction and seek advice on all matters outside usual scope of practice. 15. Practice in a manner consistent with the requirements of the relevant professional registration requirements and relevant registration board. 16. Follow guidance of the National Standards for Mental Health Services. 17. Provide supervision and support to students as required. 18. Other duties commensurate with current skills and experience as agreed with the Program Leader, Manager, General Manager or the Chief Executive Officer. Position Description Connecting 2 Community – Intensive Case Management Key selection criteria Qualifications Qualifications in Mental Health Social Work, Mental Health Occupational Therapy, Mental Health Nursing or qualifications in Community Mental Health Support Demonstrated skills, experience and/or understanding of: Minimum 2 years’ experience in a mental health or community service Demonstrated experience of working with clients with complex needs including psychosocial disability Knowledge of the welfare and mental health service networks in the Grampians region Demonstrated attention to detail with well-developed administrative and organisational skills to effectively manage high volumes of work and determine priorities, meet targets and deadlines. Resilience, including the ability to adapt and respond to change and cope with challenges Ability to maintain confidentiality at all times. Demonstrated knowledge and application of computer software, including Microsoft Office and the Internet. High level verbal and written communication skills that enable effective and appropriate communication with a broad range of people at all levels. Demonstrated ability to contribute to positive workplace cultural and practices and ability to work as part of a multi-disciplinary program area. Practical skills in facilitating groups, including the ability to prioritise different needs within a group.
