North Dorset Locality is looking for a Lead Health and Social Care Co-ordinator to join our team. This role is an exciting and developing role across the county. It is pivotal to bring services together in order to provide seamless care for frail and vulnerable individuals in our locality and it is an exciting opportunity to be at the forefront of developments in community services You will need to have excellent leadership and organisational skills. You will be a skilled communicator, as the role is integral to the development of integrated and coordinated services. You will manage a team of 5 other Health & Social care coordinators (H&SC). You will need to develop systems to ensure all H&SCC work programmes are aligned and have the same outcomes. You will be responsible for their development and performance. A good level of IT experience/computer skills is required as you will be managing data on a daily basis. You would need to have previous experience in a health or social care environment and have excellent administration skills to communicate between primary care, community services and hospitals, local authority, acute trusts and voluntary services. You will work alongside clinicians, patients, carers, statutory and voluntary agencies connected to the care of the individual patient to proactively support an effective assessment and the development of an integrated personal plan of care, to assist in avoidance of unplanned admissions and facilitate timely hospital discharge. The hours of work will be 37.5 over 5 days which will include weekends and bank holidays. Full driving licence and access to a car for work is essential as you will need to travel to meetings. Employment in this post requires a Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £40 and the Trust will recover this cost in instalments from your first 3 salary payments. This will only apply to candidates who are new to the Trust and not internal staff. Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge. It is likely that you'll be based in Shaftesbury Hospital.
