We have started a journey to transform our services to better promote the independence of our customers and to develop strengths based approaches to hospital discharge & care and support. We are looking for motivated and flexible individuals who will help to support the shaping of the future of Adult Social Care. This project aims to address these issues identified above by supporting health and social care systems to reduce the time people spend in hospital once their acute care has come to an end and helping to prevent hospital admissions. We are looking to appoint Service Coordinators to join us in the Hospital and Intermediate Care Team. These are full time opportunities and you will be required to work weekends on a rota basis. We uphold Human Rights and ensure that individuals remain central; we empower vulnerable adults to make choices and to maintain their own autonomy wherever possible. The team works in genuine partnership with people, their families and carers. We ensure that vulnerable and/or disadvantaged people are supported appropriately; they will make decisions that are in accordance with relevant policy guidance. They will deploy the correct use of legislation aimed at maintaining the independence of vulnerable adults and assist them to maximizing their own abilities and networks of support. We are committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expect all staff to share this commitment.
