The Support Planning and Brokerage team plan and arrange services for adults with care and support needs. This includes; Home Care, Residential and Nursing Care, Extra Care Housing, Community Support, Supported Living, Day services, Direct Payments, and other services for people with a wide range of needs. The Brokerage team sits within the Strategic Commissioning unit of Adult Care and Support. The post is full time – 37 hours per week, pay scale BG12 (job shares considered). We work closely with Care Providers, Social Workers and Adult Social Care teams, Finance and Contracts colleagues, Health Service colleagues and other partners. We also work closely and in partnership with Acute Hospitals to facilitate timely and appropriate discharges. The Support Planning and Brokerage team are a team of 27 people who are based at City Hall and work in other offices around the City including the hospitals. There is a Team Manager, 2 Deputy Managers and a Senior Broker, who support at team of Support Planning Co-ordinators. In the role of Deputy Team Manager you will provide regular supervision to 8 brokers, offer support and professional consultation on complex cases with team members, and support the delivery of a high quality brokerage service by working with brokers to maintain a positive team culture and consistency of practice. You will be required to deputise for the Team manager, as appropriate and departmental priorities. You will assist the Team Manager in providing high quality services to service users, families and carers in accordance with relevant legislation and policy You will operate as the senior professional within the team and deputise for the Team Manager in their absence as appropriate.
