We are seeking a highly experienced Clinical Nurse to lead the clinical team, co-ordinate a multi-disciplinary approach and deliver competent clinical care to our Community Clients based in the South Metro Region. Reporting to the Regional Manager of Community, you will be responsible for the implementation of a client directed model of care which focuses on empowerment of the client to maximise quality of life, promote healthy active ageing and facilitate identification and achievement of client goals. This role is offering full time hours between Monday and Friday with the requirement to be rostered on call (allowance payable). To be considered for this role you will need to be flexible with working times dependant on client needs. We will consider applicants from regions within a close proximity to Port Kennedy and surrounding suburbs in the Metro South Region. Key Responsibilities: Undertake and document relevant assessments that support client on-boarding and ongoing care in alignment with the Aged Care Quality Standards Provide support on clinical and health related matters to ensure that care plans are developed, implemented and evaluated with a holistic person centre approach Where required refer and collaborate with other clinicians and health care professionals to facilitate a multi-disciplinary approach to care in order to achieve effective outcomes for clients Ensure nursing services are effectively and efficiently planned, provided and charged to client records accurately Undertake regular client reviews and evaluate the effectiveness of clinical services being delivered to clients Lead, coach and mentor a team of clinicians ensuring that all clinical qualifications and competencies are maintained within the operational standards and annual performance and development reviews are undertaken Ensure all protocols and audits with regards to infection prevention and control, clinical incidence, complaints and quality improvement are followed and conducted in accordance with the operational requirements and work to embed new policies, protocols and procedures within the community setting. Ensures the Occupational Safety and Health (WA) 1984 Act and policies are complied with at all times. To be successful in this role you will have the following attributes: Essential Willingness to uphold the Mission, Vision and Values of The Bethanie Group Holds current registration as a Registered Nurse with AHPRA A minimum of 3 years’ experience in community, acute or primary health care experience Demonstrated experience in critical thinking, problem solving and use of initiative to develop and ensure effective person centred care and safe client outcomes Knowledge of applying Continuous Quality Improvement process and principles Demonstrated ability and experience in providing education, mentoring and competency assessment for health care team members Strong computer skills and experience with client database systems Knowledge of OSH legislation and requirements Current driver’s licence Knowledge in Aged Care with experience in home packages and Community common care standards highly desirable Desirable Relevant post graduate qualification Working with Bethanie At Bethanie we are committed to being an employer of choice by creating a supportive and compassionate environment for our people, customers and residents.
